Medicare’s Biggest Little Secret”

For the first time ever Medicare is making Medical Savings Accounts available to virtually all beneficiaries during the current open enrollment season. The program could be a huge boon for beneficiaries because it eliminates the need for MediGap coverage, provides a way of paying for non-Medicare covered services, limits out-of-pocket exposure, and offers the opportunity to save-up for future expenses. Working as Intended: What we have learned about Consumer Driven Health Care

Five years into the experiment with Consumer Driven Health Care, the data shows they are “working as intended.” All of the empirical evidence shows they are changing patient behavior, which is lowering costs and improving care, and resulting in a sizzling rate of adoption in the market. The negative information that has been widely touted in the press is based on three very flawed public opinion surveys.

Economists often say, “the market has failed in health care,” but this paper argues that if it has failed it is because of 100 years of endless tinkering by various levels of government. The paper walks through the history of health care regulation, beginning with The Early Years – 1900 – 1929; The Formative Era, 1930 – 1949; The Growth Era, 1950 – 1965; The Regulatory ERA, 1965 – 1980; The Competitive Era, 1981 – 1999; and finally The Consumer Era beginning in 2000.

Medicare is in crisis and to date little has been done to address it. Old strategies, such as cutting provider payments, will do little to solve the problem, and may make it worse by driving the best physicians out of Medicare altogether. Ironically the Medicare Modernization Act of 2003 authorized all Americans, except those on Medicare, to try a “consumer directed” approach that has the potential to lower costs by changing patient behavior. It is time for Congress to allow Medicare beneficiaries to join the movement to consumer empowerment.
